In the “the easier you can make their jobs, the better” department today, we have this:

a 10x dose is probably the most common hospital mistake. That’s why at our hospital they’ve changed the way that orders are written, so that those mistakes are less likely to happen, Good thing it was tylenol, and not something more dangerous, since a tylenol OD is fairly treatable if you know it’s tylenol.
Funny though, that it happened twice on something that’s so routine. Nurses on the maternity/newborn wards give tylenol post-circumcision everyday. I wonder if it was a new nurse or a new package for the tylenol (which I’m guessing was in drop form, she’d have had to give the kid an entire meal volume to get 10x down...) or something more sinister…
